The lower-ranked entity beat the higher-ranked one. The record and the ladder disagree.

Moro vs Son Goku

Two wins over the same Saiyan inside one arc, both against the ladder, and neither moves it. The first margin came partly out of the loser: on New Namek Moro fed on the Saiyans' energy as they fought. The second did not. Goku dodged the drain, but ultra instinct -Sign- burned through his stamina while Moro fought on power taken from planets he had eaten earlier. That later bout is the closer of the two, and the instinctive state Goku brought to it was an early, unstable version of the one he is ranked from. Placing him higher is a judgement about peaks, and both of these bouts come before his.

He eats the life of a planet and gets stronger by it, which makes him one of very few threats here whose power grows by a mechanism the story actually specifies rather than by transformation. Entirely manga — the arc he headlines has never been animated, so a reader who has only watched the show has not met him.

The longest depicted record anybody here has, and he is fifth because several of the entries above him have scored on him directly. Ultra instinct is the real thing — a state the narration describes as moving without deciding to, and which the panels back up by having him evade things he is not looking at. The screen and the page agree about him more consistently than about anyone else in this franchise.

Son Goku on the Dragon Ball ladder

What this does not settle

2 results, each cited, and the tally counts every one of them the same. That is its rough edge: a win at one side's lowest weighs as much as a win at its peak, which is why a level score goes to the latest result rather than to a judgement about which fight mattered. It is a record of meetings, not a ranking of its own.

The ladder above it is argued from everything either of them has been shown doing; this page is argued from these 2 citations.

Where the two disagree, the disagreement is the point, and it stays visible rather than being resolved by whichever of them happens to be on this page.
